Intake Form, Policies & Terms of Service

Prior to commencing any sessions, clients are required to complete and sign an intake form and acknowledge all policies and terms of service.

Here is a list of our terms of service. Before we begin, I want to ensure that you understand them fully. If there's anything you'd like to discuss, please don't hesitate to contact me.

Payment and Cancellation Policy

The following are the policies regarding the 24-hour cancellation policy, no-shows, and payments.

  • Sessions are prepaid only. There are no IOU or negative balances on your account.

  • There are no refunds for prepaid sessions. Unused sessions will apply as credits only.  

  • A 24-hour notice in the event of cancellations if required. If sessions are canceled within a 24-hour period or a no-show, the session will be considered a forfeit and the session will not be rescheduled as credit. 

  • For new clients, I require a commitment of 12 weeks minimum. This is a program to create change and new healthy habits and this requires a time commitment. 

    For new clients, I recommend sessions every week or every 2 weeks. 6-8 session bundles are the most cost-effective options and you do not have to complete a session every week, they will just need to be paid upfront and taken every other week at a minimum.

    This policy pertains to the initial package for new clients. New clients will engage in more in-depth comprehensive goal setting, follow-up work, and reading materials, which necessitate additional time between sessions. *For existing clients, the subsequent sessions will primarily serve as check-ins for existing clients who have successfully completed their program. Additional sessions will involve supplemental work issued on an as-needed basis.

  • For existing clients: 
    3 Session Bundles should be used within a 6-week period.
    6 Session Bundles should be used within a 12-week period. 
    8 Session Bundles should be used within a 16-week period.

  • Sessions unused for an extenuating amount of time are subject to forfeit depending on the circumstances.

Don't forget to request your intake form below. Intake forms must be submitted prior to conducting sessions. 

If you already paid for sessions, no problem. Sessions will be scheduled once the intake forms are complete.
